Agreement to be Included in the Volunteer Participant Registry of the Children's Nutrition Research Center
Purpose of the Registry
The Volunteer Participant Registry is a list of individuals who are interested in learning more about research studies done by Children's Nutrition Research Center (CNRC) researchers. Your inclusion in the Registry will not automatically enroll you/your family in a study, and you will only be contacted about studies which are all voluntary. Participation in any CNRC study will require that you provide informed consent to participate in that study.
If you would like for you and/or your child(ren) to be notified about current or future research studies at the CNRC that are recruiting participants, please:
1) carefully read the information in this Agreement,
2) provide your permission to be contacted about research opportunities by signing the Agreement, and
3) complete the Registration survey.
If you fill out the Registration survey in the next section, you are agreeing to be included in the Registry. Your identifying information will be shared with CNRC researchers strictly for research study recruitment purposes.
Examples of studies to which you may be invited to participate
Examples of studies you may be invited to participate include: children's nutrition, diet-related physiology, adiposity/weight status, obesity prevention, behaviors associated with energy balance (e.g., physical activity, sleep, sedentary behaviors, screen media, parenting, etc.), dietary intake, how diets impact digestive diseases and/or health, host and/or microbial metabolism, and maternal (pregnancy, lactation, post-partum) nutrition, metabolism, physiology of lactation, or adiposity/weight status.
Information to be collected
Upon signing this Agreement, the Registration Survey page will load and you will be asked to provide your contact information (such as phone number(s), mailing and email addresses) and answer demographic questions about you and/or your child(ren) (such as name, date of birth, sex at birth, gender, ethnicity, race).
Level of information security
Though there is always a theoretical risk of breach of confidentiality, we have taken steps to ensure that all of your information remains confidential and the privacy and safety of your information will be strictly maintained.
Participant rights
Your inclusion in the Registry is completely voluntary and your decision to be included or not will not affect any of your rights or status with any previous, current, or future CNRC studies. You have the right to withdraw your agreement to be included in the Registry at any time. It will not cost you to be included in the Registry and we will not pay you to be included. Though, if you participate in a research study at the CNRC, you may be compensated for your time and effort for that study.
Revoking your Agreement
If you revoke (take back) this Agreement, your information will be deleted from the Registry and you/your family will not be contacted for future CNRC studies. To revoke this Authorization, contact the Registry Manager, Noemi Islam, via email CNRC.VPR@bcm.edu or phone 713-798-7002. You may also contact Ms. Islam if you have any other questions or concerns.
