Family CARE Portfolio Request (formerly the Plan of Safe Care)

The Family Care Portfolio (also known as the Plan of Safe Care) is a tool designed to support pregnant people and families with complex social needs. It helps families to Coordinate care among multiple providers, Advocate for themselves, Record and store important information, and feel Empowered to take charge of their health and wellbeing.

The goal of a Family CARE Portfolio is to strengthen the family, promote healthy pregnancies and family health, and keep child(ren) safely at home. By utilizing this tool, families can demonstrate and communicate their strengths, needs, and accomplishments.

Who should be involved?

A Family CARE Portfolio should be developed with the family in coordination with all providers involved in their care. This may include health, social or community service providers. Anyone can introduce the Family CARE Portfolio to a family.

Target Populations

Any family can use the Family CARE Portfolio. It is most beneficial for pregnant people and families with complex social needs such as those that:

need help juggling life tasks
are engaged in multiple systems of care
are involved with the child welfare system (or worry about it)
live in a chaotic home environment
need support organizing and planning
struggle with substance use disorder(s)

Providers are encouraged to use the tool with any family they feel could benefit.
